Mattanawcook Jr. High School

Health Access Network will be holding voluntary vaccination clinics for both influenza and COVID vaccines at Ella P. Burr, Mattanawcook Jr. High School, and Mattanawcook Academy on Thursday, September 29^th.

The vaccines are available to your child at no cost up until their 19^th birthday.  Insurance will be billed if your child is 19 years or older.  It will be important to bring a copy of their insurance card that day.

Flu Mist is not available.

Please complete and return the permission form(s) below if you would like your child to be vaccinated. There are two separate forms, one for the influenza vaccine and one for the COVID vaccine. Please fill out the form(s) for the vaccine you would like your child to receive and make sure it is the correct form based on your child’s age. If you need a hard copy of certain forms, please contact MJHS at 794-8935 or stop and see us.
